Infant Social Resume
This form tells us a little bit about your child including their likes, dislikes, habits, and routines.  This should be completed for every child in The Zoo Crew.

Toddler & Up Social Resume
This form tells us a little bit about your child including their likes, dislikes, habits, and routines.  This should be completed for every child in each of the following rooms:  Little Sprouts, Happy Campers, Whoville, and Pirates Cove.

Medication Permission Form
This form should be completed if you need Early Einsteins to administer prescription or non-prescription medications for a period of one week or less.  One form per child should be completed, if applicable.

Diapering Products Form 
This form needs to be completed before Early Einsteins can administer ANY kind of diapering products, including but not limited to diaper rash ointments.

Infant Rolling Over Form 
This form should be completed if you allow Early Einsteins to leave your child on their stomach while sleeping if they independently roll onto their stomach.  Without this signed consent, we will need to position them on their back again.

Parent Consent for Swaddling an Infant This form should be completed if you allow Early Einsteins to swaddle your child while they are sleeping until they are able to roll over. Without this signed consent, we will not be able to swaddle them.
